Position Overview We are seeking a dedicated, compassionate, and skilled Per Diem Physical Therapist to join our homecare team! You'll have the opportunity to work in a supportive, collaborative environment with access to state-of-the-art equipment, cutting-edge treatment approaches, and a diverse caseload. Our practice is committed to empowering both our patients and our clinicians, ensuring you can focus on what matters most—helping people regain mobility, independence, and quality of life. Key Responsibilities Evaluate, diagnose, and develop individualized treatment plans for patients with a variety of musculoskeletal, neurological, and post-surgical conditions. Provide evidence-based therapeutic interventions, manual therapy, and functional training. Educate patients and families on home exercise programs, injury prevention, and self-management strategies. Document patient progress and maintain accurate, timely records in our AI-powered EMR system that reduces documentation time by up to 85%. Collaborate with referring physicians, other healthcare professionals, and internal team members to ensure continuity and quality of care. Contribute to our culture of learning through mentorship, case discussions, and ongoing professional development. Qualifications Doctor of Physical Therapy (DPT) or equivalent from an accredited program. Current New York State Physical Therapist license (or eligibility for licensure). Strong interpersonal skills and a passion for patient-centered care. Ability to work effectively both independently and as part of a multidisciplinary team.
