Early Intervention Physical Therapist - Mineola, NY Job Summary Rising Stars, a leading provider of Early Intervention services, is seeking a licensed Physical Therapist to join our team in Mineola, NY. As an Early Intervention Physical Therapist, you will play a vital role in delivering evidence-based, family-centered therapeutic support to infants and toddlers with developmental delays and disabilities. Our team is committed to supporting children and families in achieving meaningful developmental outcomes. Responsibilities Provide Early Intervention physical therapy services to infants and toddlers (birth-3) and their families in natural environments, such as homes, schools, and daycares Develop and implement individualized treatment plans to promote the child's developmental progress Collaborate with service coordinators, interdisciplinary professionals, and families to ensure comprehensive care Conduct initial and ongoing assessments to monitor the child's progress and adjust treatment plans as needed Maintain accurate and timely documentation of services provided Communicate effectively with families, service coordinators, and interdisciplinary professionals to ensure seamless care Requirements Valid New York State Physical Therapy license Strong clinical competence in delivering Early Intervention services Experience in Early Intervention is preferred Excellent professional judgment, effective communication skills, and the ability to manage documentation and scheduling requirements Reliable transportation Ability to collaborate effectively with families, service coordinators, and interdisciplinary professionals Compensation and Benefits Hourly rate: $85.00-$93.00, commensurate with experience Flexible hours If you are a motivated and dedicated Physical Therapist looking to make a difference in the lives of young children and their families, we encourage you to apply for this rewarding opportunity.
