Job Description

A leading skilled nursing facility in Long Island is seeking a motivated and compassionate Physical Therapist to join our rehabilitation team! We are dedicated to enhancing each resident's independence, mobility, and overall well-being through innovative therapy programs and personalized care.
Position Overview:
We're looking for a PT who is passionate about helping others regain strength and confidence. In this role, you'll collaborate with a team of healthcare professionals and play an important part in supporting residents on their journey toward improved function and quality of life.
Key Responsibilities:
Conduct thorough evaluations, screenings, and individualized treatment plans. Deliver high-quality rehabilitative therapy focused on helping residents improve daily functioning and mobility. Document treatments, progress notes, and clinical tasks promptly and accurately. Educate residents, families, and staff on therapy techniques, mobility strategies, and care recommendations. Provide guidance and oversight to Physical Therapy Assistants as needed.
Requirements:
