    Skilled Nursing / Rehabilitation About the Facility The Grand Rehabilitation and Nursing at Barnwell is part of The Grand Healthcare System, providing long-term care and short-term rehabilitation services. We focus on personalized care to help residents achieve optimal independence and wellness. Position Summary The Physical Therapist evaluates, treats, and guides residents to improve mobility, relieve pain, and enhance functional independence. This role collaborates with the interdisciplinary team to deliver comprehensive resident care. Primary Responsibilities Assess residents' conditions and develop individualized physical therapy plans Guide residents through exercises, hands-on therapy, and use of PT equipment Improve resident mobility, function, and independence Educate residents on therapy expectations and recovery stages Monitor progress and adjust treatment as needed Supervise physical therapy assistants when required Maintain a safe, clean, and organized therapy environment Adhere to infection-control policies and protocols Collaborate with interdisciplinary teams for comprehensive care Qualifications Graduate of an accredited Physical Therapy program Current New York State PT license Experience in skilled nursing or long-term care preferred Strong communication and interpersonal skills Resident-centered, compassionate approach Compensation and Benefits Competitive hourly pay: $40
