You'll evaluate patients, develop individualized plans of care, provide treatment, educate patients and families, and monitor progress throughout the course of care.Key responsibilities include:
Provide skilled physical therapy services in the patient's home. Complete initial evaluations and admission visits within required timeframes. Perform OASIS assessments and develop individualized plans of care. Evaluate patient progress and modify the plan of care as appropriate. Treat patients to relieve pain, restore or maintain function, and maximize independence. Educate patients and families regarding conditions, treatment plans, preventative measures, and safe mobility. Instruct patients and caregivers in the proper use of equipment such as walkers, wheelchairs, canes, crutches, braces, and other assistive devices. Collaborate with physicians, PTAs, nursing staff, social workers, schedulers, and other members of the interdisciplinary team. Supervise PTAs and, when appropriate, nurse assistants in accordance with applicable requirements. Promote continuity of care through appropriate admissions, transfers, and discharges. Help reduce hospital readmissions through patient education, proactive care, and appropriate visit scheduling. Communicate changes in patient condition, treatment response, and progress to the physician and appropriate agency staff. Complete accurate and timely clinical documentation in accordance with agency, Medicare, and federal requirements. Participate in case conferences, on-call responsibilities, in-service training, and required agency meetings. Maintain compliance with agency policies, procedures, and applicable laws and regulations. Maintain a professional appearance and represent the organization positively in the community.What We're Looking For Required:
Current Physical Therapist license in the state of employment. At least 1 year of Physical Therapist experience in an acute care setting . Valid driver's license. Current automobile insurance with your name listed as a driver. Strong communication and interpersonal skills. Ability to make sound clinical judgments and solve problems effectively. Strong organizational skills and attention to detail. Ability to prioritize responsibilities and manage time effectively. Knowledge of current physical therapy best practices.Preferred:
