Physical Therapist (PT) Wapakoneta Manor - Wapakoneta, OH Wapakoneta Manor is a 73-bed skilled nursing facility with strong growth potential, and we're looking for a Physical Therapist (PT) to join our team. This is a great opportunity to step into a building with opportunity, supportive leadership, and the ability to truly make an impact on patient outcomes.
Position Summary:
The Physical Therapist is responsible for evaluating residents, developing individualized treatment plans, and delivering high-quality therapy services to help residents regain strength, mobility, and independence.
Responsibilities:
Evaluate and assess residents' physical therapy needs Develop and implement individualized treatment plans Provide therapy services in accordance with physician orders Collaborate with interdisciplinary team members Maintain accurate and timely documentation Ensure compliance with all regulatory standards
Qualifications:
Licensed Physical Therapist in the state of Ohio Strong clinical and communication skills Experience in skilled nursing or long-term care preferred, but not required Ability to work independently and as part of a team
