Job Description

$10,000 Sign-On Bonus!! Summit Home Care and Hospice is searching for an experienced and motivated Physical Therapist (PT) to join our team. We're committed to providing comprehensive healthcare services in the comfort of our patients' homes to positively impact quality of life by redefining the delivery of care. Physical Therapist (PT):
Full-Time Hours:
Monday - Friday;
Standard Business Hours Estimated Salary:
$80,000 - $110,000 per
Year Sign-On Bonus:
$10,000 (Paid in Installments)
Coverage Location:
Washington Courthouse, OH & Surrounding Area Work Setting:
On the Road for Patient Visits & Training at the
Local Office or Remotely Responsibilities:
Provide physical therapy services to patients according to a written physician's plan of care and determine priority needs for physical therapy. Assess and evaluate therapeutic, rehabilitative/functional status and the patient's home. Develop, implement and direct physical therapy programs that focus on rehabilitation. Direct patients and families/caregivers in the use and care of therapeutic appliances. Report the patient's reaction to treatment or changes in their condition to their physician. Initiate physical therapy program and instruct other personnel and/or family/caregiver members in certain phases of physical therapy with which they may work with a patient. Prepare and submit clinical and progress summaries based on the attainment of goals. Participate in discharge planning for the patient. Participate in the peer consultation process and provide in-service education programs for nursing organization personnel as needed. Supervises PTAs and/or STNAs according to organization policy and state regulations. Collaborate with other team personnel to achieve well-rounded care.
What We're Looking For Required:
Active Ohio PT license Degree in Physical Therapy (Bachelor's, Master's, or Doctorate) CPR certification Valid driver's license, reliable transportation, and auto insurance Recent TB test
Preferred:
2+ years of PT experience Home health or community-based care experience Strong communication and organizational skills Ability to build rapport and lead with empathy Why Summit?
