Job Description

Physical Therapist (Home Health) -
Ohio Location/Coverage Area:
Highland, Clinton, and Fayette Counties, OH Pay:
$50.00/hourly

Weekly pay, Vision, Dental, MedicalPosition Overview We are seeking an experienced Home Health Physical Therapist (PT) to join our team and provide exceptional patient care in the home setting. The Physical Therapist will develop and implement individualized treatment plans designed to improve mobility, increase strength, enhance balance, and promote patient safety and independence.
Schedule & Productivity Hours:
40 hours per week
Daily Schedule:
Monday through Friday, 8:00 AM - 4:30
PM Productivity Expectation:
30 points per week
On-Call/Weekend Requirement:
None Key Responsibilities Provide skilled physical therapy services in patients' homes according to the established plan of care. Perform gait and balance training to improve patient mobility and reduce fall risk. Implement strengthening and therapeutic exercise programs tailored to individual patient needs. Conduct home safety assessments and provide recommendations to enhance patient independence and safety. Monitor patient progress and modify treatment plans as appropriate. Collaborate with physicians, caregivers, and interdisciplinary team members to ensure optimal patient outcomes. Complete all required clinical documentation accurately and timely. Required Qualifications Current Physical Therapist (PT) license in the State of Ohio. Minimum of 1 year of Home Health Physical Therapy experience. Strong clinical assessment and treatment skills. Excellent communication and patient education abilities. Preferred Qualifications Experience with Homecare Homebase (HCHB) documentation software. Ability to work independently while managing a patient caseload effectively. requirements.
