Job Description

Position Summary:
The Physical Therapist (PT) is responsible for evaluating residents and developing individualized treatment plans to improve mobility, functional independence, and quality of life. The PT delivers skilled physical therapy services in compliance with physician orders, facility policies, and applicable regulations, while working collaboratively with the interdisciplinary team in a skilled nursing facility setting.

Key Responsibilities:
Perform initial and ongoing assessments of residents' mobility, balance, strength, gait, and functional limitations. Develop and implement person-centered treatment plans to restore or improve physical function, reduce pain, and prevent disability. Provide direct therapy interventions, including therapeutic exercises, gait training, balance retraining, and use of physical agents or modalities. Monitor residents' progress and adjust care plans as necessary to ensure achievement of therapy goals. Participate in interdisciplinary care team meetings and contribute to discharge planning as appropriate. Educate residents, families, and caregivers on home exercise programs and safety strategies to promote independence. Supervise and guide Physical Therapist Assistants (PTAs) and therapy aides in accordance with licensure requirements. Accurately document treatment sessions, progress notes, and billing information in a timely manner. Ensure compliance with all state, federal, and facility documentation and practice standards. Support quality assurance and performance improvement initiatives as part of the rehab team.
Qualifications:
Graduate of an accredited Physical Therapy program. Current Physical Therapist license in the state of practice. Experience in SNF or geriatric rehabilitation preferred. Strong clinical, communication, and organizational skills. Ability to work both independently and as part of an interdisciplinary team.

Skilled Nursing Facility (SNF)/Long-Term Care setting May require frequent standing, bending, lifting, and assisting with resident mobility Fast-paced, team-based rehabilitation environment
Pay:
$60.00 - $65.00 per hour
