In this energetic and rewarding position, you will assess patient needs, develop personalized care plans, and implement evidence-based physiotherapy techniques to promote recovery and independence. Your expertise will support patients in inpatient hospital units, outpatient clinics, home health environments, and community care settings, ensuring comprehensive and compassionate care at every stage of their healing journey. Duties Conduct thorough patient evaluations to assess physical function, mobility limitations, and pain levels using physiology knowledge and medical terminology. Develop and implement tailored care plans incorporating therapeutic exercises, trigger point therapy, orthopedics strategies, and geriatrics techniques to meet individual patient goals. Perform physiotherapy interventions across various settings including inpatient hospital units, outpatient clinics, home health visits, and community programs. Manage discharge planning processes by coordinating with multidisciplinary teams to ensure seamless transition from hospital to home or community care. Document patient progress accurately within electronic medical record (EMR) systems while maintaining compliance with healthcare standards. Educate patients and caregivers on injury prevention, rehabilitation exercises, and home care routines to foster long-term wellness. Stay current with advancements in physiotherapy practices related to pediatrics, orthopedics, geriatrics, and acute care to deliver innovative treatment options. Experience Proven experience in inpatient hospital settings with a strong understanding of acute care protocols. Hands-on clinic experience providing outpatient physiotherapy services for diverse populations including pediatrics and geriatrics. Demonstrated success in developing effective care plans that incorporate discharge planning and home & community care strategies. Familiarity with EMR systems for documentation and patient management. Knowledge of anatomy and physiology principles essential for accurate assessment and targeted therapy interventions. Experience working within hospital environments or home health programs emphasizing patient care excellence. Additional skills in trigger point therapy, orthopedics treatments, or specialized areas such as sports medicine are highly valued.
