Job Description

The Home Health Physical Therapist plays a critical role in delivering personalized rehabilitative care to patients within their own homes, promoting recovery, mobility, and independence. This position requires assessing patients' physical conditions, developing tailored treatment plans, and implementing therapeutic interventions to address a variety of musculoskeletal and neurological conditions. The therapist collaborates closely with patients, families, and interdisciplinary healthcare teams to ensure comprehensive care and optimal outcomes. By providing education and support, the therapist empowers patients to manage their health and prevent further injury. Ultimately, this role contributes significantly to improving patients' quality of life by facilitating functional improvement in a comfortable and familiar environment. As a Physical Therapist , you will: Perform and document an initial exam, evaluating data to identify problems and determine a diagnosis prior to intervention. Plan, prepare, or carry out individually designed programs of physical treatment to maintain, improve, or restore physical functioning, alleviate pain, or prevent physical dysfunction in patients. Evaluate effects of treatment at various stages and adjust treatments to achieve maximum benefit. Applies concepts of infection control/universal precautions in performing patient care Assure continuity of quality patient care delivered with appropriate documentation Provide effective communication to patient/family, team members and other health care professionals Promote agency philosophy and administrative policies Maintain and complete all documentation within company stated guidelines. Requirements Successful completion of accredited physical therapy program and state licensure exam. Valid Physical Therapy license 1 year of home health experience preferred CPR certification Valid Driver's license Valid Auto Insurance HomeCare HomeBase experience preferred Physical Demands Must be able to stand, walk, sit, climb, crawl, squat, or kneel for extended periods. Must be able to lift, carry, pull, or push up to 100 lbs..
