Medford, Oregon Position Overview We are seeking an experienced Home Health Physical Therapist for a 13-week travel assignment serving Medford, OR and surrounding communities. This position is ideal for therapists who are passionate about delivering individualized patient care in a home health setting while working independently within a supportive healthcare system. Schedule Day Shift - 5x8

Hours Hours:

8:00 AM - 4:30 PM No weekends or on-call requirements Responsibilities Provide skilled physical therapy services to patients in home health settings Evaluate patient conditions and develop individualized treatment plans Monitor patient progress and adjust treatment plans as needed Educate patients and caregivers on therapy plans, exercises, and mobility techniques Complete accurate and timely clinical documentation using EPIC/Oasis systems Collaborate with interdisciplinary healthcare teams to coordinate patient care Manage a daily caseload of home health visits while maintaining productivity standards Requirements Minimum 1 year of recent skilled Home Health Physical Therapy experience required Oasis experience required Active PT license required (pending license considered) BCLS certification required Must have reliable personal transportation for patient visits Strong communication and independent clinical decision-making skills For more details reach at sthakur@navitashealth.com or Call / Text at 732 791 4807 - EXT 4807 .
