Day Shift | 5x8s | 8:00 AM - 4:30 PM Position Overview We are seeking an experienced Home Health Physical Therapist for a travel assignment serving East Portland and surrounding areas. This role is ideal for clinicians who are passionate about providing individualized patient care in home-based settings while helping patients improve mobility, independence, and overall quality of life. The Physical Therapist will manage a daily patient caseload, complete home visits, and collaborate closely with interdisciplinary healthcare teams to develop and implement effective treatment plans. Responsibilities Provide skilled physical therapy services in home health settings Evaluate patients and develop individualized treatment plans Perform functional mobility assessments and therapeutic interventions Educate patients and caregivers on exercises, safety, and home programs Complete timely and accurate Oasis documentation Coordinate care with physicians, nurses, and case managers Maintain productivity expectations while ensuring quality patient care Requirements Minimum 1 year of skilled Home Health Physical Therapy experience required Oasis experience required Active Physical Therapy license required BCLS certification required EPIC experience preferred Strong communication and documentation skills Ability to work independently in the field For more details reach at sthakur@navitashealth.com or Call / Text at 732 791 4807 - EXT 4807 .
