Day Shift | 5x8 Hours | 8:00 AM - 4:30 PM Job Summary "Navitas Healthcare, LLC" is seeking an experienced Physical Therapist for a Home Health assignment in the East Portland, OR area. This role is ideal for candidates with skilled home health experience who are comfortable working independently while providing high-quality patient-centered care in home settings. Required Qualifications Minimum 1 year of Skilled Home Health PT experience required BCLS Certification required OASIS experience required First-time travelers accepted Must have reliable personal transportation Strong communication and patient care skills required Key Responsibilities Provide physical therapy services to patients in home settings Evaluate patient conditions and develop individualized treatment plans Perform therapeutic exercises and mobility training Educate patients and caregivers on treatment plans and home exercises Maintain accurate documentation using EPIC and OASIS systems Coordinate care with interdisciplinary healthcare teams Monitor patient progress and adjust treatment plans as needed For more details reach at
