Full Job Description Role:
Licensed Physical Therapist focused on providing personalized, one-on-one care to older adults in their homes or senior living communities.
Responsibilities:
Conduct evaluations, develop individualized treatment plans, deliver evidence-based therapy to improve mobility, independence, and wellness. Document patient progress in EMR, collaborate with interdisciplinary teams, educate patients and caregivers, and incorporate telehealth when appropriate.
Qualifications & Skills:
Valid PT license, degree from an accredited program, basic computer and EMR proficiency, current CPR certification. Ability to perform physical tasks such as lifting, repositioning patients, and driving to patient locations. Physical endurance for community-based care.
Key Duties:
Support aging in place through therapy, promote safety, and foster strong patient relationships. Flexibility to create personalized treatment schedules and utilize virtual care to extend reach.
