Full Job Description Role:
Licensed Physical Therapist providing personalized care to older adults in private homes and senior living communities.
Key Responsibilities:
Conduct evaluations, develop individualized treatment plans, deliver evidence-based therapy to enhance mobility and independence, document in EMR, collaborate with interdisciplinary teams, educate patients and caregivers, and incorporate telehealth when appropriate.
Qualifications & Skills:
Valid PT license, degree from an accredited program, proficiency in EMR and computers, current CPR certification, and the ability to meet physical demands such as lifting, repositioning, and assisting with transfers.
