1 Description American Traveler is seeking a Home Health Physical Therapist with at least 1 year of skilled home health experience and active OR and WA licensure for a 13-week day shift contract serving Hood River, OR and surrounding areas. Details Home health setting serving Hood River, OR and surrounding areas Day shift schedule, 5x8 hours (8:00-16:30); 4x10 and 3x12 schedules are not available Up to 6 patient visits per day at a 1:1 ratio; standard productivity expectation is 30 points per day No weekend or on-call requirements Charting is completed via Epic Remote Client (laptop) and Rover (phone app); OASIS documentation is required Travelers are expected to float between all facilities within the state; refusal to float may result in termination of the assignment Requirements Minimum 1 year of skilled home health PT experience required Active OR and WA state PT licenses required (license required if applicable) BLS certification required OASIS experience required Epic EMR experience required Reliable personal transportation required, as travel to patient homes is necessary 2 supervisory references covering 1 year of employment within the last 3 years in a comparable specialty/setting required for consideration Full SSN and DOB required for consideration Primary COVID vaccination series plus at least one booster required Additional Information Caseload is geographically grouped to minimize drive time; occasional daily travel radius of up to ~60 miles (60-90 minutes total drive time) Mileage reimbursed at the IRS standard rate Business casual attire required; scrubs are allowed but no specific color is mandated; jeans are not permitted Former permanent employees of this health system must wait at least 3 months before returning as a traveler and cannot be within 50 miles of the submitting facility Local candidates are accepted; candidates within a 50-75 mile radius are considered local and may be accepted at the same rate Minimal RTO is strongly preferred for this position First-time travelers are welcome to apply Pre-assignment and annual online modules are required and completed on the traveler's own time
