We act like owners in all we do We're looking for a Physical Therapist who not only delivers excellent care—but also brings these values to life. ️ Position Summary As a Home Health Physical Therapist , you will provide personalized, one-on-one care to patients in their homes—helping them regain mobility, independence, and quality of life. This is a field-based role offering flexibility, autonomy, and the opportunity to build meaningful patient relationships. ✅ Key Responsibilities Patient Care Evaluate and treat patients recovering from injury, illness, or surgery Develop and implement individualized plan of care Improve patient mobility, strength, balance, and functional independence Provide interventions focused on fall prevention and safety Care Coordination Collaborate with physicians, nurses, and interdisciplinary team members Adjust treatment plans based on patient progress Participate in case conferences and care planning Patient & Caregiver Education Teach patients and families exercises, safety strategies, and mobility techniques Empower patients to manage their conditions at home Documentation & Compliance Complete accurate and timely clinical documentation (OASIS experience preferred) Follow all Medicare home health regulations and Oregon state requirements Quality & Outcomes Support improved patient outcomes and satisfaction Reduce rehospitalizations and promote safe recovery at home Qualifications Required Licensed Physical Therapist (PT) in Oregon (or eligible) Graduate of an accredited Physical Therapy program Valid driver's license and reliable transportation Ability to work independently in a field-based setting Preferred Prior home health experience Familiarity with OASIS and Medicare documentation Experience serving rural or geographically spread communities
