Job Description

Job Summary We are seeking a Physical Therapist Home Care professional for a 13-week contract opportunity in the Chester, PA and the Spring Garden Philadelphia areas. This role is ideal for a licensed PT who enjoys working independently in a home care setting while helping patients improve mobility, strength, safety, and overall quality of life. This Physical Therapist Home Care assignment offers a consistent weekday schedule, full-time hours, and the opportunity to support patients in their own homes across assigned service areas. Candidates who value patient-centered care, clear communication, supportive clinical collaboration, and meaningful one-on-one treatment will find this opportunity rewarding. Key Responsibilities Provide in-home physical therapy evaluations, treatments, and patient education based on individualized care plans. Help patients improve mobility, balance, strength, endurance, transfers, and functional independence. Document visits, progress, goals, and treatment outcomes accurately and in a timely manner. Communicate with patients, families, caregivers, and interdisciplinary care team members regarding progress and care needs. Promote safe home environments by identifying fall risks, mobility barriers, and appropriate therapy interventions. Manage a daily home care caseload while maintaining professional standards and patient-centered service.
Compensation and Benefits Pay rate:
$55 to $56 per hour.
Contract length:
13 weeks.
Schedule:
8:00 AM to 4:30 PM.
Hours:
40 hours per week.
Assignment area:
Home care coverage for zip codes 19013 and 19130.
