We are seeking a compassionate and skilled Physical Therapist, Full Time or PRN, to join our team at Brinton Manor Nursing and Rehabilitation Center. As a Physical Therapist, you will provide high-quality, personalized care to our patients in a fast-paced and dynamic environment. Our mission is to make each day count, and we are looking for a dedicated and empathetic professional to help us achieve this goal.
Responsibilities:
Provide physical therapy services to patients Develop and implement individualized treatment plans to achieve patient goals and outcomes Conduct thorough evaluations and reassessments to ensure optimal patient care Work collaboratively with other members of the interdisciplinary team to provide comprehensive care Document patient progress and maintain accurate records Participate in patient education and discharge planning
