Job Description
Home Health Physical Therapist (PT) - PRN / Per Diem Philadelphia, Montgomery, Bucks, Delaware & Chester Counties We are currently seeking a PRN Physical Therapist to provide home health visits throughout the greater Philadelphia region. Responsibilities
- Perform PT evaluations in patients' homes
- Develop and implement personalized treatment plans
- Provide therapy focused on mobility, strength, balance, and fall prevention
- Monitor patient progress and adjust care plans as needed
- Educate patients and caregivers on home exercise programs and safe mobility
- Document visits in the agency EMR system
- Collaborate with nurses, physicians, and the interdisciplinary care team Requirements
- Active Pennsylvania Physical Therapist license
- Home health experience preferred but not required
- Strong assessment and patient education skills
- Ability to work independently in a home setting
- Reliable transportation for travel between patient visits Position Details
