Job Description
Job Summary We are seeking a passionate, dedicated, and licensed Physical Therapist to join our growing clinical team in the Lancaster, PA area. In this role, you will deliver exceptional, patient-centered care to help individuals restore mobility, reduce pain, and achieve their ultimate functional goals. Responsibilities Evaluations & Care Plans:
Perform comprehensive initial patient evaluations to diagnose dysfunctions and establish individualized, evidence-based treatment plans.Treatment Delivery:
Administer targeted therapeutic interventions focusing on strength, balance, manual therapy, and functional mobility.Patient & Family Education:
Educate patients, caregivers, and families on safety strategies, home exercise programs, and long-term wellness.Compliance & Documentation:
Maintain thorough, accurate, and timely electronic medical records (EMR) according to state regulations and insurance compliance.Interdisciplinary Collaboration:
Coordinate care seamlessly with physicians, physical therapist assistants (PTAs), occupational therapists, and administrative staff.Progress Tracking:
Continually reassess patient response to interventions and modify treatment plans to optimize clinical outcomes. Requirements Valid licensure or certification as a licensed Physical Therapist in the relevant jurisdiction. Proven experience practicing in a physical therapy role within outpatient clinics, or home health environments. Strong knowledge of physiology, anatomy, medical terminology, and clinical documentation standards. Demonstrated ability to develop effective treatment plans for patients with complex needs such as orthopedics, neurology, geriatrics or post-surgical recovery. Experience working with electronic health records (EHR) or electronic medical records (EMR) systems for documentation and treatment tracking. Excellent client communication skills with the ability to motivate and educate diverse patient populations. Familiarity with discharge planning processes and working collaboratively with healthcare teams to optimize patient outcomes.
