Duties Assess patients' physical conditions and develop personalized treatment plans aligned with clinical best practices and patient goals Provide hands-on therapy interventions such as therapeutic exercises, trigger point therapy, and gait training to promote recovery and functional independence Conduct discharge planning to ensure smooth transitions between care settings and ongoing patient support Maintain accurate clinical documentation in compliance with healthcare standards and electronic health records (EHR) systems Skills Strong understanding of clinical documentation standards and electronic health records (EHR/EHR systems) management Skilled in discharge planning, treatment plan development, and patient rehabilitation strategies Ability to collaborate seamlessly with healthcare professionals across various disciplines Excellent client communication skills to educate and motivate patients throughout their recovery journey Join us at REHAB1 where your expertise will help transform lives through compassionate care!
