Job Description

About us We are currently looking for a dependable Physical Therapist interested in a flexible PRN/contract position with the opportunity to consistently treat approximately 2-15 hours per week, with opportunities for additional work. Wolff Rehab is an independently owned mobile outpatient physical therapy practice serving patients throughout Luzerne County. We specialize in providing individualized, high-quality rehabilitation services directly in patients' homes, allowing for a more personalized and meaningful treatment experience. This role is ideal for a clinician who values autonomy, enjoys one-on-one patient care, and wants to move away from the high-volume clinic setting. Therapists at Wolff Rehab have the ability to manage their own schedules while delivering skilled outpatient care in the home environment.
Primary Responsibilities:
Provide skilled outpatient physical therapy services in patient homes Perform evaluations and develop individualized treatment plans Progress therapeutic interventions based on patient presentation and goals Monitor functional improvements and communicate updates as needed Maintain timely and accurate electronic documentation Coordinate care with physicians, caregivers, and other healthcare providers
Preferred Qualifications:
Graduate of an accredited Physical Therapy program Active Physical Therapy license Reliable transportation and valid driver's license Strong communication and organizational skills Ability to work independently while managing a flexible caseload New graduates and experienced clinicians are encouraged to apply
