Job Description
Tender Touch Rehab Services is seeking a compassionate Physical Therapist Floater to provide high-quality, evidence-based rehab across multiple skilled nursing and long-term care facilities. In this field-based role, you'll evaluate and treat senior patients, develop individualized treatment plans, and focus on improving function, independence, and quality of life. You will collaborate closely with an interdisciplinary team in a supportive, patient-centered culture that values dignity, compassion, and respect. Responsibilities Provide evidence-based physical therapy to seniors across multiple skilled nursing and long-term care facilities as a floater Perform evaluations, develop individualized treatment plans, and deliver interventions to improve function, independence, and quality of life Document assessments, goals, treatments, and outcomes accurately within EMR systems and per regulatory requirements Collaborate with an interdisciplinary team, including nursing and rehabilitation staff, to coordinate comprehensive patient-centered care Educate patients, families, and caregivers on home exercise programs, safety, and mobility strategies Adapt quickly to varying caseloads, facility processes, and team dynamics while maintaining clinical excellence Participate in quality improvement, outcomes tracking, and best-practice initiatives within Tender Touch Rehab Services Engage in ongoing professional development, mentorship, and training to enhance clinical skills and career growth Required Skills Geriatric physical therapy Skilled nursing facility (SNF) rehab Functional mobility and gait training Therapeutic exercise prescription Balance and fall-prevention programs Transfer and wheelchair training Care planning and documentation (EMR) Pain management techniques Interdisciplinary care coordination Knowledge of Medicare/SNF regulations
