Adecco Healthcare & Life Sciences is hiring a Travel Physical Therapist in Selinsgrove, Pennsylvania! This role is on-site / in-person. Please review the details below and with an updated resume for consideration.
Type:
Three-month contract, with possibility of an extension
Schedule:
Monday - Friday | 8:00 AM - 4:30 PM
Location:
Selinsgrove, PA 17870
Pay:
$56 to $59 per hour
Primary Responsibilities:
- Complete start of care visits, physical therapy evaluations, and routine home health visits
- Assess and evaluate patient care needs related to mobility, endurance, pain, and functional status
- Develop and implement individualized treatment plans for home health patients
- Perform therapeutic interventions to improve patient strength, mobility, and overall well-being
- Monitor patient progress and adjust treatment plans as needed
- Document patient care using Home Care Home Base (HCHB) electronic medical records
- Collaborate with physicians, nurses, caregivers, and interdisciplinary healthcare teams
- Travel between patient homes within assigned coverage areas in Ling County, PA
- Maintainpliance withpany policies, state regulations, and patient care standards
Qualifications:
- Active Physical Therapist license required
- Previous Home Health experience required
- Experience using Home Care Home Base (HCHB) EMR preferred
- Strongmunication and patient care skills
- Prior experience with LHC Group or similar home health settings preferred
