Three-month contract, with possibility of an extension
Schedule:
Monday - Friday | 8:00 AM - 4:30 PM
Location:
Selinsgrove, PA 17870
Pay:
$56 to $59 per hour
Primary Responsibilities:
  • Complete start of care visits, physical therapy evaluations, and routine home health visits
  • Assess and evaluate patient care needs related to mobility, endurance, pain, and functional status
  • Develop and implement individualized treatment plans for home health patients
  • Perform therapeutic interventions to improve patient strength, mobility, and overall well-being
  • Monitor patient progress and adjust treatment plans as needed
  • Document patient care using Home Care Home Base (HCHB) electronic medical records
  • Collaborate with physicians, nurses, caregivers, and interdisciplinary healthcare teams
  • Travel between patient homes within assigned coverage areas in Ling County, PA
  • Maintainpliance withpany policies, state regulations, and patient care standards
Qualifications:
  • Active Physical Therapist license required
  • Previous Home Health experience required
  • Experience using Home Care Home Base (HCHB) EMR preferred
  • Strongmunication and patient care skills
  • Prior experience with LHC Group or similar home health settings preferred
