Provide therapeutic interventions to improve mobility, strength, and overall physical function. Educate patients and their families on treatment plans, exercises, and strategies for recovery. Monitor patient progress and modify treatment plans as necessary to achieve optimal outcomes. Collaborate with healthcare professionals to ensure coordinated care and effective discharge planning. Maintain accurate documentation of patient assessments, treatments, and progress in compliance with medical standards. Skills Strong knowledge of anatomy and physiology relevant to physical therapy practices. Proficient in medical terminology to effectively communicate with healthcare teams. Experience in creating and implementing effective care plans tailored to individual patient needs. Ability to conduct discharge planning that ensures continuity of care post-treatment. Familiarity with home health settings and the unique challenges they present for patient rehabilitation. Experience working with geriatric populations, understanding their specific needs for rehabilitation. Proficiency in providing inpatient physical therapy services within a clinical environment. Excellent interpersonal skills to foster positive relationships with patients and their families.
