Full Job Description Job Summary:
We are seeking a compassionate and skilled Physical Therapist (PT) and Physical Therapist Assistant (PTA) to join our Home Health and Hospice team serving patients in the East Bay Area. In this role, you'll work one-on-one with patients in the comfort of their homes to help them regain mobility, manage pain, and improve their quality of life. You'll collaborate with a multidisciplinary team to develop and implement individualized care plans.
Key Responsibilities:
Conduct in-home physical therapy evaluations and assessments Develop and implement individualized treatment plans based on patient needs and goals Educate patients and caregivers on therapy exercises and safe mobility techniques Monitor and document patient progress in accordance with agency and regulatory guidelines Coordinate care with nurses, occupational therapists, physicians, and other healthcare professionals Provide ongoing updates to the clinical team and revise treatment plans as needed Ensure compliance with all state, federal, and agency policies and procedures
Requirements:
Current California Physical Therapist (PT) license Graduate of an accredited Physical Therapy program Valid Driver's License and reliable transportation Ability to work independently and manage a flexible schedule Strong communication and documentation skills CPR certification (BLS)
Preferred Qualifications:
Experience with OASIS documentation Bilingual (Spanish/English) a plus Minimum 1 year of experience preferred, ideally in home health or geriatrics
