Job Description

Category Homehealth - PT Job Type Contract Exciting opportunity available for an experienced Physical Therapist to deliver high-quality home health services for adult patients. This contract position offers 8-hour day shifts, from 8:00 AM to 5:00 PM, Monday through Friday, with a regular 40-hour workweek. There are no on-call, call-back, floating, or weekend rotation requirements. Holiday work may be scheduled according to service needs.
Key Qualifications:
Minimum 1 year of recent physical therapy experience, including proficiency in home health settings BLS certification and an active, valid state PT license (Bachelor's degree required) Charting experience with Homecare Homebase EMR and strong competency with OASIS-E assessments Valid driver's license and proof of current insurance Experience serving adult and geriatric populations with chronic disease, wound care (including wound vac, pleurx), IV therapy, assessment, and disease management/teaching Ability to work independently and chart maintenance notes in a timely, accurate manner
Primary Responsibilities:
Complete approximately 6 home health visit "points" per day with attention to patient safety, goal attainment, and quality care standards Deliver evidence-based therapy, evaluate patient progress, and educate patients and families in their home environments Manage a caseload focused on adults managing chronic diseases, wounds, and other complex care needs Ensure compliance with regulatory and documentation standards via Homecare Homebase Participate in a comprehensive 5-day orientation and utilize Samsung Note 9 technology for field documentation Wear designated gray therapy scrubs during field visits
