Job Description

Category Homehealth - PT Job Type Contract Deliver high-quality patient care in a home health setting as a physical therapist, focusing on adult and geriatric clients with chronic diseases, wound care needs, and ongoing therapy requirements. This is a contract position offering an 8-hour day shift, Monday through Friday, from 8:00 am to 5:00 pm, with no on-call or weekend rotation.
Schedule:
8-hour day shifts: 8:00 am - 5:00 pm, Monday through Friday No on-call, call-back, or weekend rotation Holiday shifts as scheduled
Role Responsibilities:
Conduct home visits (average of 6 points per day) within assigned county coverage Complete comprehensive assessments using OASIS-E and provide detailed documentation in Homecare Homebase Develop and implement individualized treatment plans for patients dealing with chronic disease, post-surgical recovery, wound care (including IV, pleurx, and wound vac), and general mobility concerns Educate patients and families on disease process, preventative measures, and in-home exercises Collaborate with departmental leadership and clinical colleagues to enhance patient outcomes Utilize provided Samsung Note 9 phones and current EMR software for all charting requirements Adhere to facility policies, dress code (grey scrubs), and mileage expectations if necessary
Qualifications:
Active Physical Therapist license in South Carolina Current BLS certification Bachelor's degree in Physical Therapy or higher At least 1 year of experience, with strong proficiency in home health and adult/geriatric care Competency with Homecare Homebase (no exceptions) and OASIS-E Valid driver's license and proof of insurance Experience with wound care, chronic disease management, and maintenance notes
