Job Description
Physical Therapist (PT) | Full-Time & Part-Time All Heart Home Care | Columbia, TN All Heart Home Care is seeking compassionate and skilled Physical Therapists (Full-Time and Part-Time) to join our growing home health team in the Columbia, Tennessee area . This role provides the opportunity to deliver personalized, one-on-one care to patients in the comfort of their homes while enjoying a flexible schedule and a supportive work environment. Key Responsibilities Perform in-home evaluations to assess patient needs and develop individualized treatment plans Provide skilled physical therapy services to improve mobility, strength, balance, and overall function Monitor patient progress and modify treatment plans as appropriate Educate patients and caregivers on home exercise programs, safety measures, and injury prevention Complete accurate and timely clinical documentation in accordance with company policies and state regulations Collaborate with physicians, nurses, case managers, and other healthcare professionals to ensure coordinated patient care Travel to patient homes within the assigned service area Qualifications Doctor of Physical Therapy (DPT) or equivalent from an accredited program Active Tennessee Physical Therapist license Current CPR certification Valid driver's license, reliable transportation, and current automobile insurance Strong clinical assessment and communication skills Ability to work independently and manage time effectively Previous home health experience preferred, but not required Available Positions Full-Time Physical Therapist Part-Time Physical Therapist About All Heart Home Care At All Heart Home Care, we are committed to providing high-quality, compassionate care that allows patients to thrive in the comfort of their own homes.
