JOB SUMMARY
Directs and provides therapeutic care of patients in their place of residence. The PT is responsible for assessing ability or disability, relieving pain, restoring motion, improving functional skills, and obtaining or maintaining maximum performance within the patient's capabilities. Responsible for individualized evaluation using standardized testing procedures to create measurable outcomes. The home health physical therapist optimizes the patient's quality of life through prescribed exercise, hands-on care and patient/caregiver education. The home health physical therapist must possess clinical competence and leadership. They must be adaptable and able to work with all age groups. Collaborates with the treatment team to provide optimal care. Provides supervision of the Physical Therapy Assistants. On call is required for potential weekend or holiday visit coverage.
JOB REQUIREMENTS
Current License as a Physical Therapist in TN Required 1 Year of Related Experience Preferred
