Job Summary We are seeking a dedicated and compassionate Physical Therapist to join our dynamic healthcare team. In this role, you will be responsible for assessing, planning, and implementing individualized treatment plans to help patients regain their mobility, reduce pain, and improve their overall quality of life. You will work closely with a multidisciplinary team to deliver exceptional patient care across various settings, including outpatient clinics, inpatient hospitals, home health, and community care environments. Your expertise in physiology, anatomy, and medical terminology will be vital in developing effective rehabilitation strategies tailored to each patient's unique needs. Responsibilities Conduct comprehensive patient assessments to determine functional limitations and develop personalized treatment plans aligned with clinical documentation standards. Implement evidence-based physiotherapy interventions such as trigger point therapy, orthopedics techniques, neurology treatments, and geriatrics care to facilitate recovery. Collaborate with healthcare professionals including physicians, nurses, occupational therapists, and social workers to coordinate patient care and discharge planning. Manage electronic health records (EHR) or electronic medical records (EMR) systems accurately to document treatment progress and ensure compliance with healthcare regulations. Provide patient education on injury prevention, rehabilitation exercises, and community care options to promote ongoing recovery outside of clinical settings. Assist in discharge planning by evaluating patient progress and recommending appropriate home health or outpatient services for continued care. Participate in outpatient rehabilitation center activities or inpatient hospital rounds to support patient rehabilitation goals across diverse clinical environments. Skills Proven experience practicing in a physical therapy role within outpatient clinics, hospitals, or home health settings. Strong knowledge of physiology, anatomy, and medical terminology relevant to physical therapy practices. Expertise in developing comprehensive care plans that incorporate discharge planning and treatment plan development strategies. Skilled in working with geriatric patients, pediatrics, orthopedics, neurology cases, and patients requiring specialized therapies such as trigger point therapy. Familiarity with outpatient experience and inpatient hospital experience to adapt treatments accordingly. Excellent client communication skills to effectively educate patients about their treatment options and progress. Proficiency in managing clinical documentation standards and electronic health records (EHR/EMR systems) for accurate recordkeeping. $44.61 - $48.42 per hour
