is seeking a compassionate, motivated, and dependable Physical Therapist (PT) to provide home and community-based therapy services for individuals receiving Tennessee Department of Disability and Aging (DDA) Waiver services throughout Martin, TN and surrounding communities . This is an excellent opportunity for a therapist who enjoys flexible scheduling, one-on-one patient care, and improving quality of life in the home environment. Position Type PRN / Independent Contractor (1099) Flexible Schedule Competitive Per-Visit Compensation Service Area Martin, TN Dresden McKenzie Union City Greenfield Sharon Gleason Weakley County Obion County Additional surrounding communities as needed Responsibilities Complete comprehensive physical therapy evaluations. Develop individualized Plans of Care (POCs). Provide skilled physical therapy interventions in the patient's home or community setting. Educate caregivers and direct support staff on safe mobility, transfers, positioning, fall prevention, adaptive equipment, and therapeutic exercises. Complete accurate and timely electronic documentation. Communicate with physicians, case managers, support coordinators, and the interdisciplinary team. Participate in re-evaluations and progress updates as required by DDA guidelines. Maintain compliance with Tennessee regulations and

policies. Qualifications Graduate of an accredited Physical Therapy program. Current Tennessee Physical Therapist license in good standing. Current CPR certification. Valid driver's license and reliable transportation. Professional liability insurance (or ability to obtain). Excellent communication and documentation skills. Home health or developmental disability experience preferred, but not required.
