Role Description We are seeking a licensed Physical Therapist to deliver personalized, in-home therapy services designed to improve mobility, strength, balance, functional independence, and overall quality of life for older adults and individuals recovering from illness, injury, or surgery. As an independent contractor, you will have the flexibility to manage your own schedule while making a meaningful impact in the lives of clients in their homes. Responsibilities Conduct comprehensive physical therapy evaluations in clients' homes Develop individualized treatment plans based on each client's goals, functional abilities, and clinical needs Provide therapeutic exercises, mobility training, strength and balance interventions, and fall-prevention strategies Monitor client progress and adjust treatment plans as appropriate Educate clients and family members on home exercise programs, mobility techniques, and injury prevention Collaborate with caregivers, family members, and other healthcare professionals to support client outcomes Maintain accurate and timely documentation of evaluations, treatment sessions, and client progress Adhere to all applicable state regulations, professional standards, and HIPAA requirements Qualifications Graduate of an accredited Physical Therapy program Minimum of three (3) years of clinical experience preferred Experience working with older adults, home-based care, geriatrics, orthopedic rehabilitation, or post-acute care preferred Strong clinical assessment and treatment planning skills Excellent interpersonal, communication, and client education skills Ability to work independently and manage a flexible caseload Strong organizational and time-management abilities Reliable transportation and willingness to travel to client homes within the service area Licenses, Certifications & Insurance Requirements Current and unrestricted Physical Therapist (PT) license in the State of Tennessee Current CPR/BLS certification preferred Professional liability (malpractice) insurance or ability to obtain coverage prior to providing services Valid driver's license and proof of automobile insurance Ability to successfully pass a background check consistent with company requirements
