Job Description
CareRite Centers seeks a compassionate Physical Therapist to provide skilled therapy services in a post-acute and long-term care setting. In this role, you will assess residents, develop evidence-based treatment plans, and deliver individualized interventions to improve mobility, function, and quality of life. You will collaborate closely with an interdisciplinary team, document in the EMR, and educate residents and families. Responsibilities Evaluate residents and develop individualized PT treatment plans Deliver evidence-based therapeutic interventions to improve mobility and function Document assessments, goals, and progress in EMR accurately and timely Collaborate with nursing, rehab, and medical teams on care plans Educate residents and families on exercises, safety, and discharge plans Participate in care conferences and quality improvement initiatives Ensure compliance with regulatory, documentation, and billing standards Supervise and mentor PTAs and rehab support staff as appropriate Required Skills Physical therapy evaluation Gait training Neuromuscular re-education Therapeutic exercise prescription Manual therapy techniques Balance and fall prevention Wound care/edema management basics EMR documentation Care planning in SNF/post-acute settings Interdisciplinary team collaboration
