Physical Therapist (PT) - PRN Burleson, TX | Park Bend Rehabilitation & Skilled Nursing •Pay starting at $55/hr and increases dependent upon experience.• Park Bend Rehab is seeking a dedicated and compassionate Physical Therapist (PT) to join our exceptional in-house rehabilitation team in Burleson, Texas. Position Summary The Physical Therapist will evaluate, develop, and implement individualized treatment plans for residents and patients experiencing physical limitations, mobility impairments, pain, or functional deficits. The ideal candidate will possess strong clinical skills, excellent communication, and a passion for improving patient outcomes through high-quality rehabilitative care. Responsibilities Evaluate residents' physical conditions, mobility limitations, strength, balance, endurance, and functional abilities Develop individualized treatment plans designed to restore function, reduce pain, improve mobility, and maximize independence Implement therapeutic exercises, manual therapy techniques, gait training, balance activities, and other skilled interventions Monitor patient progress and modify treatment plans as clinically appropriate Educate patients, family members, and caregivers on treatment plans, exercises, safety, and preventative care strategies Collaborate with physicians, nursing staff, and interdisciplinary team members to ensure coordinated patient care Maintain accurate and timely documentation in compliance with company policies and state/federal regulations Ensure all treatments are delivered in accordance with the established plan of care Participate in care planning meetings and attend required facility meetings as directed by the Director of Rehabilitation Maintain compliance with all facility policies, procedures, and professional standards Qualifications Active and valid Texas Physical Therapist license Graduate of an accredited Physical Therapy program Skilled nursing facility (SNF) or long-term care experience preferred Knowledge of Net Health documentation system preferred Strong clinical, communication, and organizational skills Ability to manage patients with varying diagnoses and personalities Why Join Park Bend Rehab?
