In this vital role, you will deliver expert physiotherapy care directly to patients in their homes, promoting recovery, independence, and improved quality of life. Your expertise will help bridge the gap between inpatient care and community living, ensuring seamless care plans tailored to each individual's needs. This position offers an exciting opportunity to make a meaningful difference in patients' lives while working in a collaborative and supportive environment. Duties Assess patients' physical conditions through comprehensive evaluations, including physiology and anatomy knowledge, to develop personalized care plans. Implement evidence-based physiotherapy techniques such as trigger point therapy, orthopedics, geriatrics, and pediatrics to facilitate recovery and pain management. Coordinate discharge planning and collaborate with multidisciplinary teams to ensure continuity of care across inpatient, outpatient, hospital, and home health settings. Conduct patient care activities that include therapeutic exercises, mobility training, and functional assessments within the home or community environment. Document all patient interactions accurately using EMR (Electronic Medical Records) systems, ensuring compliance with healthcare regulations. Educate patients and caregivers on injury prevention, proper use of assistive devices, and self-care strategies to promote long-term wellness. Participate in ongoing training related to acute care practices, medical terminology updates, and new physiotherapy techniques to stay current in the field. Skills Strong knowledge of physiology and human anatomy essential for accurate assessments and effective treatment planning. Proven experience with home & community care environments, including home health services and outpatient clinics. Familiarity with inpatient hospital settings and acute care procedures enhances your ability to adapt treatments across various environments. Excellent patient care skills combined with the ability to communicate complex medical terminology clearly and compassionately. Proficiency in Wellsky EMR for documentation and patient record management. Ability to develop comprehensive care plans that incorporate discharge planning and follow-up strategies. Experience with specialized therapies such as trigger point therapy, orthopedics, geriatrics, pediatrics, and outpatient physiotherapy.
