Per Diem / PRN Physical Therapist Avir at Montana 10350 Montana Avenue El Paso, TX 79925 Position Summary Avir at Montana is seeking a dedicated and compassionate Per Diem / PRN Physical Therapist (PT) to join our rehabilitation team. The Physical Therapist will provide skilled evaluation and treatment services to patients with a variety of mobility, strength, balance, and functional limitations. The goal is to help patients improve physical function, achieve greater independence, reduce the risk of injury, and enhance their overall quality of life. Essential Responsibilities Evaluate patients' physical and functional abilities and develop individualized treatment plans. Provide skilled physical therapy interventions based on each patient's diagnosis, condition, and rehabilitation goals. Treat mobility, strength, balance, coordination, endurance, gait, and functional limitations. Provide therapeutic exercises, functional mobility training, gait training, and other appropriate skilled interventions. Monitor patient progress and modify treatment plans as clinically appropriate. Complete accurate and timely evaluations, treatment notes, progress reports, and discharge documentation. Collaborate with physicians, nursing staff, occupational therapists, speech-language pathologists, and other members of the interdisciplinary team. Educate patients, families, and caregivers regarding exercises, mobility techniques, safety precautions, and home programs. Participate in care planning and interdisciplinary meetings as needed. Maintain compliance with applicable federal and state regulations, facility policies, and professional standards. Provide coverage on a flexible PRN/per-diem basis according to facility and patient needs. Qualifications Current Physical Therapist license in the State of Texas . Graduate of an accredited Physical Therapy program. Current CPR certification, if required by facility policy. Previous experience in skilled nursing, rehabilitation, long-term care, or a related healthcare setting preferred. Strong clinical, communication, organizational, and documentation skills. Ability to work independently and effectively as part of an interdisciplinary healthcare team. Dependable, flexible, compassionate, and committed to providing high-quality patient-centered care.
