Job Description
Job Summary We are seeking a dedicated and energetic Physical Therapist (PT) to join our dynamic healthcare team. In this role, you will provide expert rehabilitative care to the geriatric population. Your primary focus will be developing personalized treatment plans that promote recovery, improve mobility, and enhance quality of life. You will collaborate closely with healthcare professionals to ensure comprehensive patient care and discharge planning, utilizing your extensive physiotherapy knowledge and clinical expertise. Responsibilities Assess patients' physical conditions through detailed evaluations, including anatomy and physiology considerations Develop and implement individualized treatment plans aligned with patient goals and clinical best practices Deliver therapeutic interventions such as manual therapy, trigger point therapy, and mobility exercises tailored to each patient's needs Monitor patient progress regularly, adjusting treatment strategies as necessary to optimize outcomes Document all patient interactions accurately in electronic health records (EHR) systems following clinical documentation standards Collaborate with multidisciplinary healthcare teams—including physicians, nurses, and occupational therapists—to coordinate care plans and discharge strategies Educate patients and their families on home exercise programs, community resources, and self-care techniques to support ongoing rehabilitation Skills Strong knowledge of anatomy, physiology, neurology, orthopedics and geriatrics to inform effective treatment approaches Proficiency in outpatient clinic experience, inpatient hospital settings, home health environments, and outpatient rehabilitation centers Familiarity with medical terminology relevant to physical therapy practice and documentation requirements Experience working with electronic health records (EHR) or electronic medical records (EMR) systems for accurate recordkeeping Ability to develop comprehensive care plans that incorporate discharge planning and patient education Excellent client communication skills to foster trust and motivate patients throughout their recovery journey Knowledge of physiotherapy techniques such as trigger point therapy and treatment plan development for diverse conditions including musculoskeletal injuries and neurological disorders Join us in delivering exceptional patient care by applying your expertise in physiotherapy within a supportive environment that values your skills.
