PRN Physical Therapist Home HealthJoin a team that values flexibility, autonomy, and patient-centered care. We are seeking a compassionate PRN Physical Therapist to provide exceptional in-home therapy services that help patients regain mobility, reduce pain, and improve overall function and quality of life.
As a Physical Therapist, you will: Perform and document comprehensive evaluations to identify patient needs and develop individualized treatment plans Plan and provide therapeutic interventions to restore function, improve mobility, relieve pain, and prevent disability Monitor patient progress and modify treatment plans to maximize outcomes Maintain timely, accurate documentation per company guidelines Communicate effectively with patients, families, physicians, and interdisciplinary team members Follow infection control and universal precaution standards Support agency philosophy and commitment to quality patient care

Requirements:
Graduate of an accredited Physical Therapy program Current Physical Therapy license Minimum 1 year home health experience preferred CPR certification Valid drivers license and auto insurance HomeCare HomeBase experience preferredPhysical Demands:
Must be able to stand, walk, bend, squat, kneel, and lift/push/pull up to 100 lbs as needed.Benefits
