Job Summary We are seeking a dedicated and skilled PRN (as-needed) Physical Therapist to join our home health care team. The ideal candidate will possess a strong background in physiotherapy, with experience working with diverse patient populations including geriatric, pediatric, neurological, and orthopedic patients. This role involves developing individualized treatment plans, delivering patient-centered care, and collaborating effectively with healthcare professionals to promote optimal recovery and functional independence. Responsibilities Conduct comprehensive assessments of patients' physical abilities and develop tailored treatment plans aligned with clinical standards. Provide therapeutic interventions such as manual therapy, trigger point therapy, and exercise programs to facilitate patient rehabilitation. Collaborate with physicians, nurses, and other healthcare team members to coordinate care and ensure seamless discharge planning. Maintain accurate clinical documentation in accordance with medical terminology standards and electronic health records (EHR) systems. Educate patients and their families on treatment techniques, home exercises, and community resources to support ongoing recovery. Manage patient progress through regular evaluations and adjust treatment strategies as necessary to meet individual goals. Ensure compliance with all clinical documentation standards, safety protocols, and healthcare regulations across inpatient, outpatient, hospital, and home health settings. Qualifications Valid state licensure as a Physical Therapist with practicing experience in a clinical setting. Demonstrated expertise in outpatient clinic environments, inpatient hospital care, or home health services. Experience working with geriatric patients, pediatrics, neurology cases, orthopedics, or outpatient rehabilitation centers is highly desirable. Strong knowledge of physiology, anatomy, medical terminology, and physiotherapy techniques. Proficiency in electronic health records (EHR) or electronic medical records (EMR) systems for documentation management. Excellent client communication skills with the ability to educate patients effectively about their care plans. Familiarity with discharge planning processes, care plan development, and working within multidisciplinary teams.
