The ideal candidate will have experience in home health settings and possess a strong understanding of anatomy and physiology. This role involves working with patients of various age groups, mainly geriatrics, to develop and implement effective treatment plans that promote recovery and enhance mobility. Responsibilities Conduct comprehensive evaluations of patients to assess their physical condition and needs. Develop individualized care plans based on patient assessments, goals, and treatment objectives. Implement therapeutic interventions to improve patients' physical function, strength, and mobility. Educate patients and their families about treatment options, exercises, and recovery processes. Collaborate with other healthcare professionals to ensure coordinated patient care. Document patient progress and modify treatment plans as necessary based on ongoing assessments. Participate in discharge planning to ensure a smooth transition from therapy to home or other care settings. Stay updated with the latest medical terminology, techniques, and best practices in physical therapy. Qualifications Degree in Physical Therapy from an accredited program. Valid state licensure or eligibility for licensure as a Physical Therapist. Strong knowledge of anatomy, physiology, medical terminology, and rehabilitation techniques. Experience in outpatient settings preferred; acute care experience is a plus. Familiarity with discharge planning processes and geriatric care is advantageous. Excellent communication skills with the ability to educate patients effectively. Strong organizational skills with attention to detail in documentation and care plans.
