Job Overview We are seeking a dedicated and compassionate Physical Therapist Assistant to join our healthcare team. The ideal candidate will work under the supervision of a licensed Physical Therapist to assist in the implementation of patient care plans. This role is essential in providing high-quality rehabilitation services to patients across various settings, including skilled nursing facilities, clinics, and home health environments. Duties Assist Physical Therapists in developing and implementing individualized care plans for patients. Provide direct patient care, including therapeutic exercises and modalities. Monitor patient progress and report changes to the supervising Physical Therapist. Educate patients and their families on treatment plans and home exercise programs. Participate in discharge planning to ensure a smooth transition for patients. Maintain accurate documentation of patient treatments and progress notes. Collaborate with interdisciplinary teams to optimize patient outcomes. Support the physical therapy team in maintaining a clean and organized work environment. Experience Knowledge of human physiology and anatomy is essential for effective patient care. Experience working with diverse populations, including geriatrics, pediatrics, and acute care patients. Familiarity with discharge planning processes and skilled nursing facility protocols is advantageous. Previous experience in home health or clinic settings is preferred but not required. Strong communication skills to effectively interact with patients, families, and healthcare professionals.
