Duties Conduct comprehensive patient evaluations to determine functional limitations and rehabilitation needs Develop and implement individualized treatment plans focused on patient recovery and mobility enhancement Perform therapeutic interventions including manual therapy, trigger point therapy, and exercise programs tailored in geriatric populations. Manage discharge planning processes to ensure seamless transition of care across settings Document all patient interactions accurately in electronic health records (EHR) systems following clinical documentation standards Collaborate closely with healthcare professionals including physicians, nurses, and occupational therapists to optimize treatment outcomes Educate patients and their families on physiotherapy exercises, injury prevention, and home care strategies to promote long-term wellness Skills Strong knowledge of anatomy, physiology, and medical terminology relevant to physical therapy practice Proven experience working with geriatric patients. Skilled in developing effective care plans and treatment strategies aligned with clinical standards Proficiency in managing electronic health records (EHR) and electronic medical records (EMR) systems for accurate documentation Excellent client communication skills to motivate patients and explain treatment procedures clearly Ability to work collaboratively within multidisciplinary teams to enhance patient rehabilitation efforts Experience in discharge planning for skilled nursing facilities.
