Job Description
VIGEO Home Health is seeking a Virginia-licensed Physical Therapist for part-time or PRN home health visits in Northern Virginia, with current coverage needs centered around Centreville and nearby communities including McLean, Arlington, and Vienna. - Flexible part-time/PRN scheduling based on your submitted availability and accepted assignments.
- $110
- 160 per visit/hour, based on visit type and confirmed during the hiring process.
- Mileage reimbursement according to agency policy.
- Paid onboarding, paid training, and approved administrative time for required non-visit work.
- Patient assignments grouped geographically whenever census and scheduling permit.
- Practical support with scheduling, documentation, care coordination, and compliance.
- A W-2 role with the autonomy of home-based practice and the backup of an active care team.
- A broader VIGEO Health network spanning home health, outpatient physical therapy, and companion care across Northern Virginia.
You will:
- Complete PT evaluations, reassessments, care-plan updates, and discharge planning in patients' homes.
- Provide skilled interventions focused on gait, balance, mobility, strengthening, safety, edema management, and functional independence.
- Educate patients and caregivers on home exercise programs, fall prevention, device use, and safe mobility.
- Coordinate meaningful changes in condition and treatment recommendations with physicians and the interdisciplinary care team.
- Supervise and support PTAs when assigned and permitted.
- Complete timely documentation in the agency EMR consistent with Medicare home health standards.
- Participate in role-specific orientation, training, and competency requirements. Required qualifications
- Current Virginia Physical Therapist license in good standing.
- Degree from an accredited Physical Therapy program.
- Current CPR certification.
- Valid driver's license, reliable transportation, and required automobile insurance.
- Ability to provide in-home visits in the Centreville / Northern Virginia service area.
- Independent field judgment with prompt communication to patients, caregivers, physicians, and the care team.
- Ability to complete onboarding and competency requirements before independent patient care. Preferred qualifications
- Home health or Medicare-certified home health experience.
- OASIS experience.
- WellSky/Kinnser or comparable home health EMR experience.
- Geriatric, orthopedic, neurological, fall-prevention, edema/lymphedema, or post-hospitalization experience.
- Comfort with digital documentation and mobile clinical workflows.
