Home Health Physical Therapist (PT) We are seeking a compassionate and motivated Home Health Physical Therapist to join our growing team in Warrenton, VA. In this role, you will provide high-quality physical therapy services to patients in their homes, helping them improve mobility, manage pain, and achieve greater independence.

Responsibilities:

Evaluate patients and develop individualized treatment plans Provide skilled physical therapy services in the home setting Monitor patient progress and adjust treatment plans as needed Educate patients and caregivers on exercises and safety practices Collaborate with physicians, nurses, and interdisciplinary team members Maintain accurate and timely documentation

Qualifications:

Current Physical Therapist license in the state of practice Graduate of an accredited Physical Therapy program Home health experience preferred but not required Strong communication and organizational skills Valid driver's license and reliable transportation
