Our client is currently seeking a Home health

Registered Nurse Schedule:

Monday to Friday, 8:00 AM - 4:30 PM Position Overview We are seeking an experienced and compassionate Home Health Physical Therapist to provide high-quality patient care in a home-based setting. The ideal candidate will evaluate, develop, and implement individualized treatment plans to help patients improve mobility, restore function, and achieve maximum independence in their daily lives. Key Responsibilities Conduct comprehensive physical therapy evaluations in patients' homes. Develop and implement individualized treatment plans based on patient needs and physician orders. Provide therapeutic exercises, mobility training, balance and gait training, and patient/family education. Monitor patient progress and adjust treatment plans as appropriate. Complete timely and accurate clinical documentation. Collaborate with physicians, nurses, and interdisciplinary care teams to ensure optimal patient outcomes. Maintain compliance with agency policies, state regulations, and home health standards. Required Qualifications Active Virginia Physical Therapist License. Previous Home Health Physical Therapy experience. Experience using HomeCare HomeBase (HCHB) for documentation. Proficiency with PointClickCare.
