Responsibilities Physical Therapists at DHHC screen, evaluate, re-evaluate, and treat patients as well as coordinate and communicate treatment activities with our interdisciplinary team members and the patient.
The PT will:
• Evaluate the patient's need for physical therapy and develop an appropriate treatment plan. • Carry out appropriate treatment programs in accordance with agency policies and professional standards of care. Communicate with all members of the health care team. Document evaluations, treatment goals, and objectives. Teach patients and family members the rationale for treatments and procedures. Qualifications
REQUIRED QUALIFICATIONS
Must be a licensed Physical Therapist in Vermont and New Hampshire or hold a compact license. Graduate of an accredited school in Physical Therapy
PREFERRED QUALIFICATIONS
One year of experience as a PT in a home health care setting is preferred Required Licensure/Certifications Unencumbered PT license in VT and NH or compact. Driver's license, minimum requirements on car insurance, and a clean driving record
