1 Description American Traveler is seeking a Physical Therapist for a Home Health assignment requiring at least 1 year of recent home health experience and
HCHB EMR
experience. Details Home health setting covering Snohomish and Skagit County (Mount Vernon) and Island territory Territory extends up to a 60-mile radius from the hub office Day shift schedule, 8-hour shifts, Monday through Friday, 8:00 AM to 5:00 PM No weekend or on-call requirements Patient population primarily geriatric Average caseload of 5-7 patients per day with 6 visits daily Productivity requirement of a minimum of 30 points weekly Uses
HCHB EMR
system Requires in-office visits weekly or biweekly for team meetings and supply/admission packet pickup Requirements Minimum 1 year of recent home health physical therapy experience required BLS certification required OASIS experience required
HCHB EMR
experience required Experience with geriatric patient population Must be flexible with territory assignments Timely documentation and communication required License required if applicable Additional Information Main responsibilities include home exercise program development and patient/family education Scrubs required in any color; no jeans permitted iPad provided for documentation Mileage reimbursed at the IRS standard rate First-time travelers will not be considered for this position Local candidates accepted at the travel rate
